Dental Coverage Summary

Delta Dental administers two dental plan options:

Dental With Orthodontics
Dental No Orthodontics

Both dental plans offer two cleanings a year, with the same providers but different pricing. Review the chart below for details when using Delta in-network providers — Delta Dental PPO or Delta Dental Premier.

Benefit Cost

SmileWay Program

The SmileWay program allows for two additional cleanings (four total) per year for anyone with high-risk health conditions that can benefit from additional oral health care.

No ID Card Needed

You do not need a member ID card to get care with Delta Dental. When you schedule an appointment, tell the office your name and date of birth. If you prefer to have a paper or electronic ID card, create a member account at deltadentalins.com to view or print your ID card.

2027 SINOT STD Bi-Weekly Premiums — Puerto Rico

Seguro por Incapacidad No Ocupacional Temporal (SINOT) is a state-mandated STD program required for Puerto Rico Coworkers.
SINOT premiums are deducted from your paychecks on an after-tax basis.

* In the HSA Plus medical plan, you must satisfy the medical plan deductible before the coinsurance percentage shown in the chart will apply.

Prescription drug coverage is provided with BCBSTX medical plans.

BCBSTX Medical Plans — In-Network

* Each covered person has their own $6,000 deductible and out-of-pocket maximum. Once one person reaches $6,000, the plan pays 100% of that person's in-network covered costs for the rest of the year, even if the family total of $17,400 has not been met.
** Includes medical and prescription drug costs.
‍*** No one person pays more than $6,000 in a year, even on family coverage. Once one person reaches $6,000, the plan pays 100% of that person's in-network covered costs for the rest of the year, even if the family total of $12,000 has not been met.

Kaiser Medical Plan — In-Network

* You must use a Kaiser provider or facility in the Kaiser network for services to be covered.‍

Hospital Indemnity — Benefit Amounts

* If you are enrolled in Hospital Indemnity coverage, and if child coverage is effective before birth or elected within 60 days after birth, your newborn will receive the same cash benefits as you. If you don’t enroll in child coverage within that timeframe, you’ll receive a one‑time $300 newborn benefit at birth.

* Eligible domestic partners and their qualified children may enroll in health care coverage. Their coverage is taken after‑tax, and the value of the employer‑paid premium is treated as taxable income, which means a higher contribution amount on your pay slip.

2027 Medical Plan Bi-Weekly Premiums — Kaiser California

* Rates may be higher if you or your spouse/domestic partner self-identify as a tobacco user. Coworkers enrolled in the Kaiser medical plan are not eligible for the Wellness Credit.

Bi-weekly Coworker contributions are based on salary tiers to comply with the Hawaii Prepaid Health Care Act.

* Your 2027 medical plan rates and salary-based benefits use your base salary as of October 1, 2026. Pay changes after that date will not change your 2027 rates. Coworkers eligible for commission will use a two-year average of base pay plus commissions.
** Rates may be higher if you or your spouse/domestic partner self-identify as a tobacco-user and/or did not complete the wellness requirements.

Plan not available to Hawaii or Puerto Rico residents.
